The flu season is approaching. Every year, we see many cases of influenza from November through springtime.
We recommend that you get your annual flu vaccine early.
Call our office to schedule an appointment. We are giving the flu vaccine to all patients over 6 months of age and parents/caregivers.
Children under 9 years of age who have not received a flu vaccine in the past will need a booster four weeks later.
This year’s vaccine includes the H1N1 vaccine. We have both the injectable flu shot and the nasal mist available.
The nasal mist flu vaccine may be given to anyone over 2 years of age who does not have a history of asthma and who is not pregnant or immunocompromised.

Alternative Caregiver Consent Form (this link not set but is ready)
In order to designate another adult to bring your child into our office for medical care in your absence, you must complete and sign an Alternative Caregiver Consent Form. Except for life threatening emergencies, we are not able to treat your minor child unless he or she is accompanied to our office by a parent, legal guardian or a adult designated on this form.
Release of Medical Records FROM Glendale Pediatrics
You may use this form to request a copy of your child's medical records to be picked up from our office. Please be aware that there is a fee for duplication of medical records per patient. An additional fee will be charged if the chart is exceptionally large or if you request that the records be mailed. Please be advised that we do not fax medical records. Records will be provided to you within 15 days from the date that Glendale Pediatrics receives your written request.
